I hereby authorize the veterinarian to examine, prescribe for, and/or treat the above described pet. I assume responsibility for all charges incurred in the care of my pet. I also understand that all fees are due at the time services are rendered. I also authorize Tamberly Animal Hospital to take pictures of my pet(s) to help with their online presence. (Pets may be featured on our social media pages and website).
